Thank you for choosing Kentucky Orthopedic Associates. We appreciate your business. The following are a few policies for services at our practice. If you have concerns or questions about our policies, please contact us at 859.737.5333.
- Your are required to check in when you arrive.
- New patients will have to fill out a registration sheet, so please arrive early.
- We request that office visits be paid at the time of services.
- Please have your insurance cards available at the time of registration.
- Please notify us of any changes in address, phone numbers or insurance as soon as possible.
- If you are a self-pay patient, we expect payment in full at time of services unless other arrangements have been made with the office manager.
- Full clothing is required when attending our offices unless your injury has made it impossible.
- All forms are to be filled out and signed to the best of your knowledge.
POLICY REGARDING MEDICATION
Patients should discuss the need for medications or prescriptions during the office visit. Office staff cannot authorize refills. All requests must be approved by the physicians.
MEDICATION REFILLS
Requests by phone may take up to 48 hours to be approved by a provider before being called in to your pharmacy. If requesting refills by phone, please provide office staff of the name and phone number of your pharmacy.
PAIN MEDICATION
Pain Medicines are prescribed by providers in this office only for limited medical conditions and injuries, including acute fractures and post-operative pain. Pain medicines are not given for chronic conditions. Patients needing refills on pain medications may be required to pick up the prescription in person.
PRESCRIPTIONS WILL ONLY BE CALLED IN DURING BUSINESS HOURS. PRESCRIPTIONS WILL NOT BE CALLED IN AFTER HOURS OR ON WEEKENDS.
Patients are responsible for their own medication, no early refills can be called in for lost or stolen medicines.
